An off-site follow up survey conducted on July 6, 2023 found that Angel Army LLC had corrected the following deficiency cited under 28 Pa. Code, Health Facilities, Part IV, Chapter 51, Subpart A. The deficiency was cited as a result of a complaint investigation survey completed on April 19, 2023.

The deficiencies were cited as a result of a complaint investigation survey completed on April 19, 2023.
